The shoulder benefits from gentle daily movement
Move the arm through a range that feels easy. Keeping it still all day can weaken the muscles that guide the joint. Reaching may then become harder, even after the first ache settles. Slow motion and light strength work keep those muscles active.
Don't push through sharp soreness. Use less weight or make the reach shorter. Warmth before moving may make stiff motion easier, while ice can calm the ache afterward. Stop if either one makes you feel worse.
Comfortable motion is enough for today.
Soreness may start in a tendon, the joint, or the neck
Four rotator cuff tendons help keep the arm bone in its shallow socket. A tendon is the strong tissue joining muscle to bone. Years of use can leave one tendon frayed or sore. You may feel an ache in the outer upper arm. Reaching overhead, dressing, or sleeping on that side can hurt.
Arthritis wears the smooth surface where the bones meet. It can cause deep aching, stiffness, or grinding. With a frozen shoulder, the strong tissue wrapped around the whole joint becomes tight. The arm then stays stiff even when someone else tries to move it. Soreness from the neck may also travel into the shoulder and arm.
Tell the examiner how the soreness began and which movements are difficult now.
A sudden loss of strength needs a prompt exam
A slow ache without an injury often leaves time for gentle motion. Notice whether sleep, dressing, reaching, or lifting keeps getting harder. An exam becomes worthwhile when soreness keeps limiting those daily jobs. The examiner can check the joint, the cuff tendons, and the neck.
Get urgent help if a fall leaves the shoulder misshapen or you can't lift the arm. Fever with redness, heat, and swelling also needs quick care. Sweating, nausea, shortness of breath, or chest pressure with shoulder soreness can mean the trouble isn't in the joint, so call emergency services instead of waiting for a regular office visit.

The RESTORE trial - a participant-, injector- and assessor-blinded RCT of 288 adults aged 50+ with symptomatic medial knee OA (Kellgren-Lawrence 2-3) - compared three weekly intra-articular PRP injections against saline placebo, with co-primary endpoints of 12-month knee pain and medial tibial cartilage volume on MRI. PRP did not beat placebo on either. It is the single best-designed test of the specific claim that PRP changes joint structure, and it was negative.

A four-arm, multicentre, single-blind phase 2/3 randomized trial of 480 knee OA patients (KL II-IV) compared autologous bone marrow aspirate concentrate, autologous adipose stromal vascular fraction and allogeneic umbilical-cord-tissue mesenchymal stromal cells against a corticosteroid injection control. At 12 months NONE of the three orthobiologic injections was superior to another, or to the corticosteroid control, and none of the four groups showed a significant change in MRI osteoarthritis score from baseline. No procedure-related serious adverse events occurred.

In the MOON Shoulder prospective cohort of 452 patients with symptomatic, ATRAUMATIC full-thickness rotator cuff tears, physical therapy succeeded in more than 70% of patients at 10 years: only 115 (27.0%) had surgery at any point over the decade. Patient-reported outcomes improved with physical therapy and did NOT decline over 10 years in those who never had surgery. The strongest predictor of early surgery was low patient expectation of physical therapy, not tear anatomy.

The GRASP trial randomized 708 adults with a rotator cuff disorder to progressive exercise (up to 6 sessions), a single best-practice advice session, or either of those preceded by a corticosteroid injection. Over 12 months there was no evidence of a difference in Shoulder Pain and Disability Index between progressive exercise and one advice session (adjusted mean difference -0.66, 99% CI -4.52 to 3.20), and no evidence of a difference between having a corticosteroid injection and not having one.
